Nomination Requirements
Who Can be Nominated: Individuals or groups working to eliminate TB in the United States.
Eligibility: Only one nomination per person is allowed. 2024 TB Elimination Champions are not eligible.
Deadline: Submit your nomination by February 14, 2025
Instructions
Completed Nomination Form:
- Complete and submit the online .
- Include details about the nominee's contributions and impact.
- Upload a photo of the nominee using the below requirements, including a signed photo release form.
Photo of Nominee(s):
- Please only submit one photo per nomination. For groups, consider submitting a photo collage.
- The accepted file formats are PNG, JPG, and JPEG.
- Image size (based on orientation)
- Square: 660×660 pixels
- Landscape: 660×372 pixels
- Portrait: 660×858 pixels
- Square: 660×660 pixels
- Minimum resolution: 300 dpi
- Images should use the following format: Nominee Name_TB Champion.
Signed Photo Release Forms:
- Please complete and sign a photo release form.
- Required for all individuals in the photo.
- Combine multiple forms into one PDF if needed.
